CABS Special Access Form – SPA100

This form is to be used for reporting all special access billing. Circuits that are provisioned by an IXC will appear on the CABS bill for that carrier. Circuits that are billed directly to an end user are billed under a separate platform with individual invoices created for each customer. This form replaces the old SPA001 and SPA003 forms.
